ASIAN PACIFIC YOUTH PROGRAM
Provides services to Asian Pacific youth and their families. We help youth with culturally/ linguistically appropriate mental health services through counseling, crisis intervention, medication support, case management and/or community outreach efforts to increase success in school and in family interactions.
DIFFERENTIAL RESPONSE CASE MANAGEMENT
We teach families to use resources in their community and help them with parent coaching and education. Families may be referred to our Differential Response Therapeutic Services program listed below.
DIFFERENTIAL RESPONSE THERAPEUTIC SERVICES
We arrange for family therapy to help families nurture change and development. Case management is available.
ETHNIC & CULTURE SPECIFIC WELLNESS
The Ethnic and Culture Specific Wellness Program at Gardner Health Services aims to build awareness about mental health and related services and resources for the Latino and African descent communities of the Santa Clara County. The program includes outreach activities in the heart of the communities and from our centers at various locations in the county including San Jose, Alviso, and Gilroy. We hope to create awareness, decrease cultural bias and stigma about mental health challenges and participate in advancing culturally sensitive treatment and care to improve the wellbeing of our community members.
FAMILY COURT SERVICES
Family Court Services performs child custody mediation, emergency screening, and evaluation services upon court referral. We support parents to meet court orders and link them to mental health assistance.
FAMILY & CHILDREN
Provides services to children ages 6 – 18 to improve the emotional, psychological and intellectual well–being of children and bridging cultural and generational differences within families. Strengthens a client’s ability to access personal and community resources via counseling, parenting support, medication support and 24 hour on-call service.
FAMILY ENRICHMENT PROGRAM
Empowers families with children ages 0-5, to build healthy relationships through case management, therapeutic services, home visitations and parent/guardian support.
FAMILY STRENGTH BASED SERVICES
Our counselors provide services to families to reduce the risk of abuse and prevent further involvement in the foster care system. Services include counseling, parenting support and community referrals.
FIRST 5
Free resources, educational activities, and positive parenting workshops that provide support in developing the skills that children and families need to thrive. Available to families with children ages 0-5, parents, and caregivers.
FIRST 5 FAMILY COURT SERVICES
FIRST 5 Specialists at Family Court can help families get resources for children. Eligible families with a court order for supervised visitation may have these services paid by money from FIRST 5.
CHILD FULL SERVICE PARTNERSHIP
We provide intensive therapy for children ages 6-15. We combine intensive psychological therapy and psychiatric support. We help alleviate family crisis to help the child and family thrive.
INTENSIVE OUTPATIENT PROGRAM
Our psychiatrists and counselors guide high risk youth in social services and on probation to improve their daily function and complete their probation requirements. We offer comprehensive assessments, individual, family, and group counseling and medication evaluation.
STUDENT LINKED SERVICES
Our program is geared towards school aged children in the San Jose Unified School District System. Services include individual and group counseling, case management, family support, home based support, and crisis support.
THERAPEUTIC BEHAVIORAL SERVICES
Children and youth, whose behavior is severe enough to risk losing placement or hospitalization, are provided with intensive short-term treatment. Children in this program are typically already receiving mental health counseling.
TRANSITIONAL AGE YOUTH FULL SERVICE PARTNERSHIP
We provide youth ages of 16-25 and their families who are struggling with both mental health related issues as well as substance abuse problems with include individual counseling, family therapy, rehabilitation services, group therapy, and psychiatric services.
